Practice Transformation Manager

Responsibilities

• Lead the implementation and deployment of value-based care models including population health management and risk-based contracting
• Collaborate with providers and practice staff to optimize clinical and administrative workflows
• Implement and optimize practice use of technology solutions including EHRs and care management tools
• Develop and implement population health management programs for high-risk patient populations
• Partner with practices to execute strategies to achieve quality performance goals
• Facilitate care coordination among healthcare providers, patients, and families
• Develop performance measurement systems to evaluate VBC interventions
• Build strong, trust-based relationships with practice administrators and providers

Requirements

• Bachelor's degree in Healthcare Administration, Public Health, Nursing, or a related field
• 3+ years of experience in practice transformation, care coordination, or population health
• Direct experience working with physician practices in a value-based care environment
• Strong understanding of value-based care models and primary care operations
• Ability to manage multiple practice relationships simultaneously
• Proficiency in using data and performance metrics to guide interventions

Preferred Qualifications

• Clinical background or direct experience in primary or ambulatory care settings
• Familiarity with EMR systems, HEDIS measures, and population health platforms
• Experience with ACO, MSSP, or private payer value-based care program requirements
