Clinical Guide, RN

Responsibilities
- Perform clinical triage to assess patient health concerns using evidence-based protocols
- Provide clinical navigation support including condition education, care planning, and medication adherence
- Document symptoms, nursing assessments, and guidance provided accurately
- Conduct proactive outreach to high-risk and rising-risk members to assist with lifestyle changes
- Coordinate screening services and provide care management follow-up
- Act as a health advocate to direct members to high-quality providers and appropriate care settings
Requirements
- RN, ADN, or BSN degree
- Active RN license in a compact state and residency in a compact state
- 5+ years of clinical experience
Preferred Qualifications
- Experience in family medicine
- Triage nursing experience
- Care management or chronic condition support experience
- Multiple RN state licenses (CA, NY, MI, MA, MN)
