Diabetic Educator

The ideal candidate will play a pivotal role in helping individuals with diabetes achieve better health outcomes and will work closely with patients to provide education, support, and guidance on managing their diabetes through lifestyle modifications, medication management, and self-care strategies.
Responsibilities:
· Conduct comprehensive assessments of patients with diabetes to determine their educational needs and individualized care plans.
· Provide one-on-one education and counseling to patients on various aspects of diabetes management, including blood glucose monitoring, nutrition, exercise, medication administration, and lifestyle modifications.
· Assist patients in setting achievable goals for blood glucose control and overall health improvement.
· Collaborate with the healthcare team, including physicians, nurses, and dietitians, to develop and implement personalized care plans.
· Conduct group education sessions and workshops to support multiple patients in their diabetes management journey.
· Monitor patients' progress and provide ongoing support through regular follow-up appointments.
· Educate patients about potential diabetes-related complications and strategies for prevention.
· Stay up-to-date with the latest developments in diabetes management, treatments, and technologies.
· Maintain accurate and organized records of patient interactions and educational materials.
Qualifications:
· Bachelor's degree in nursing, dietetics, or a related healthcare field. A Certified Diabetes Educator (CDE) certification is preferred.
· Minimum of 5 years of experience in diabetes education and management.
· Strong interpersonal and communication skills.
· Empathetic, patient-centered approach to care.
· Proficiency in using diabetes management tools and technology.
· Ability to work collaboratively within a healthcare team.
