Nurse Practitioner

Responsibilities

• Oversee monitoring of patient vitals, symptoms, and labs in collaboration with RNs and MDs
• Manage treatment plans and medications with emphasis on GDMT for Heart Failure, Hypertension, and Type 2 diabetes
• Respond to patient escalations identified via remote patient monitoring
• Lead regular virtual check-ins with patients to review labs and medication optimization
• Ensure exceptional healthcare interactions that prioritize patient well-being
• Support internal growth efforts to scale care delivery for chronic conditions

Requirements

• 5+ years experience treating chronic diseases (CHF, Hypertension, T2D)
• Experience managing CHF with GDMT
• Active ANCC or AANP certification
• Master’s Degree as a Nurse Practitioner
• Active Compact Multi-State RN license
• Active NP licensure in at least two of: TX, MI, NC, CA, PA, NJ, CT, AL
• Stable home Wi-Fi with strong upload/download speeds

Preferred Qualifications

• Experience in a CHF bridge clinic or T2D clinic
• Experience in a startup environment
• Experience with remote patient monitoring technology
• Comfort using AI-assisted clinical tools
