Homecare Nurse

The Homecare Nurse is responsible for providing professional nursing care to patients in their homes. The role involves assessing patient (New Born or Old Patients) health conditions, administering treatments and medications, monitoring progress, educating patients and family members, and collaborating with physicians and other healthcare professionals to ensure high-quality, patient-centered care.

Key Responsibilities

Patient Care

• Assess, plan, implement, and evaluate nursing care for patients in home settings.
• Monitor patients’ vital signs, symptoms, and overall health condition.
• Administer medications, injections, intravenous (IV) therapy, and prescribed treatments.
• Perform wound care, dressing changes, catheter care, and other nursing procedures.
• Assist patients with activities of daily living when required.

Care Planning & Monitoring

• Develop and maintain individualized care plans based on physician instructions and patient needs.
• Observe and document changes in patient condition and report concerns promptly.
• Coordinate care with physicians, therapists, caregivers, and other healthcare providers.
• Ensure continuity of care through accurate record-keeping and communication.

Patient & Family Education

• Educate patients and family members on disease management, medication administration, and health maintenance.
• Provide guidance on nutrition, hygiene, mobility, and preventive healthcare practices.
• Train caregivers on safe patient handling and homecare procedures.

Documentation & Reporting

• Maintain accurate nursing records, treatment notes, and progress reports.
• Complete documentation in accordance with healthcare regulations and organizational policies.
• Report incidents, adverse reactions, and significant health changes to the supervising physician or nurse manager.

Infection Prevention & Safety

• Follow infection control procedures and standard precautions.
• Ensure safe administration and storage of medications.
• Maintain a clean and safe environment for patients.
• Identify and address potential health and safety risks in the home.

Emergency Response

• Recognize signs of medical emergencies and respond appropriately.
• Provide basic life support and emergency care within the scope of practice.
• Coordinate emergency medical services when necessary.

Qualifications

• Diploma, Associate Degree, or Bachelor’s Degree in Nursing.
• Valid Registered Nurse (RN) or equivalent nursing license as required by local regulations.
• Current Basic Life Support (BLS) certification; Advanced Cardiac Life Support (ACLS) certification is an advantage.

Experience

• Minimum 2–5 years of clinical nursing experience.
• Previous experience in home healthcare, community nursing, hospital nursing, elderly care, or chronic disease management is preferred.
• Experience with pediatric, geriatric, post-operative, or critical care patients is advantageous.

Technical Skills

• Proficiency in nursing assessments and clinical procedures.
• Knowledge of medication administration and patient monitoring.
• Familiarity with infection control standards and healthcare regulations.
• Ability to use electronic medical records and healthcare documentation systems.
